• Bring your DENTAL ID AT ALL TIMES during your visit. You need also to present other valid ID before you can avail.

• As much as possible, call any of our Dental Partners during their regular clinic hours to schedule for appointment.

• Our Dentists may need to call our Customer Service Hotline to the validate the services under your plan. Please bear with this added procedure.

• After the procedure, make sure to write your contact number and affix your signature in our Service Charge Slips​. This is to confirm the validity of the service rendered.
