Step 1: Book Your Appointment 

To minimize wait times, please schedule an appointment with one of our accredited partner clinics.

• Tip: Mention that you are an Elite Group Member when booking.

Step 2: Present Your Credentials
Upon arrival, please present the following To The Clinic Staff:

• Your Member ID (Physical or Digital ID).

• One (1) Valid Government-Issued ID for identity verification.

Step 3: Membership Verification
The clinic will contact our Customer Service Hotline to verify your plan coverage and the specific services requested. We appreciate your patience during this brief verification process.

Step 4 - Receive Your Treatment

Once your coverage is confirmed, you may proceed with your dental treatment.​

Step 5: Settle Non-Covered Services (If Any)

If you avail of services that are not included in your dental plan coverage, you will be responsible for settling the corresponding fees directly with the clinic.

Before proceeding with any non-covered treatment, the clinic will inform you of the costs for your approval.

Important: Finalizing Your Visit
After your procedure, please review the Service Charge Slip. To confirm that the service was rendered correctly:

• Verify the details of the treatment.

• Provide your current contact number.

• Affix your signature.
